Connect Academy is now welcoming student referrals for all three of our therapeutic day school programs across the greater Chicagoland area. We partner directly with school districts to provide individualized, supportive, and academically rigorous environments for students with emotional, behavioral, and developmental needs, including students with autism.

Referral Process: How to Refer a Student
We make the referral process simple and collaborative. Here's how it works:

Step 1: District Referral
A district representative completes our online referral application, identifying a student who may benefit from one of our therapeutic day programs. Refer a Student.

Step 2: Application Review
Our intake team reviews:

• The student’s IEP and evaluations
• Behavioral data and incident reports (last 4–6 months)
• Current placement details
• Referral reasoning and eligibility

Step 3: Program Placement
Based on the student’s age and needs, we recommend placement in one of the following programs:

• Connect Academy (Pre-K–8)
• Connect Academy High School (Grades 9–12)
• Pathways Academy (students with autism)

Step 4: Ongoing Partnership
We work closely with the district, family, and student to ensure a smooth transition and maintain collaboration throughout the student’s time at Connect Academy.

Note: Space is limited. A referral does not guarantee placement.

Documents Needed for Referral
Be prepared to upload the following when submitting a referral:

• Most recent IEP
• Most recent Evaluation or Re-evaluation
• Last 4-6 months of behavioral data and incident reports
• Student DOB, eligibility, current placement, and parent contact info

This information ensures accurate placement and tailored support from day one.
