WELCOME TO THE CONNECT ACADEMY

The Connect Academy is dedicated to providing comprehensive support for students with varying disabilities through our specialized therapeutic day programs. Our partnership with school districts is essential in ensuring that students receive the individualized care and educational opportunities they need to thrive. This page outlines our referral process, detailing how we work with districts to place students in the appropriate program based on their age and unique needs. REFERRAL PROCESS

At The Connect Academy, we collaborate with school districts to ensure that students with varying disabilities receive the specialized support they need. Our referral process is designed to place students in the appropriate program based on their age and specific needs.

STEP 1: ​District Referral

School districts identify students who may benefit from our therapeutic day programs and initiate the online referral application.

STEP 2: Review

Our team reviews the student's educational and behavioral needs, considering the recommendations from the district.

STEP 3: Program Placement

Program Placement: Students are placed in one of our three programs:
  • Connect Academy (Pre-K - 8th): Located in Carol Stream, Illinois, this structured therapeutic program builds strong academic and emotional foundations to support student reintegration into less restrictive settings.
  • Connect Academy High School (9th - 12th): Based in Addison, Illinois, this supportive program prepares students for life beyond the classroom through academic instruction, behavioral support, and community-based learning opportunities to support transition planning.
  • Pathways Academy (Supporting Students with Autism): Located in Lombard, Illinois, this highly individualized program serves students with autism by providing structured routines, communication supports, and therapeutic care in a nurturing environment.

STEP 4: Ongoing Support

We work closely with the referring district, families, and the student to ensure a smooth transition and continuous support throughout their time at Connect Academy.

When referring a student to Connect Academy, our specialized intake form requires a few pieces of information so we can properly assess a students fit for our organization. Please be ready with the below information and documents to ensure a smooth application process.

Required Information:
  • Student Name
  • District
  • District Contact Name, Phone, and Email
  • Student Date of Birth (DOB)
  • Grade
  • Primary Eligibility
  • Parents' Names and Contact Information
  • Current Placement
  • Reason for Referral

Required Documents:
  • Most Recent IEP
  • Most Recent Evaluation/Re-evaluation
  • Last 4-6 Months of Behavioral Data and Incident Reports
